Some young people find it hard to talk to adults about their worries, especially their healthcare worker. Some people also find it hard to think about their choices during an appointment, because they are worried.

This tool was designed to help. It has different parts for you to write down things about yourself, your worried, and what you want to happen (or don't want to happen) when you have treatment.

​

You might want to use this to remind yourself or your parent/carer OR you might want to give it to your healthcare worker so they understand how you feel and what you need.

 

There are three different versions you can use - one has a scale from 1-10 to measure worry, one has a scale with faces and and the other has a picture you can colour.

​

You can also use a version on your mobile phone or device.

​

This tool is for helping you stay CALM - it has 4 sections where you can write things about yourself.

c- is for COMFORT

What would you like to do to help yourself feel better during your appointment? You can write down your coping ideas here (if you need some ideas you can read our suggestions here)

A-is for ASKING

Are there any questions you want to ask about your treatment or what will happen? You can take a note of them here (if you want some examples of questions you might like to ask, you can read our suggestions here)

 

L-is for LEVEL OF WORRY

How worried are you feeling about your treatment? You can use our scale from 1-10 (where 10 is very worried and 1 is not worried at all) or you can colour in our picture if you prefer (the higher you colour the more worried you are)

M-is for MY CHOICES

​

What things would you like to happen during your appointment? Is there anything you don't want to happen? This might be things like:

- Do you want to watch?

- Do you want the person to hide everything?

- Would you like the person to count down before they start?

- Would you like someone to hold your hand or talk to you?

You can fill in all 4 parts, or just the ones you think will be important for you.

​

This tool is designed to be used quickly, it should only take a minute for you or someone else to read. You can use it to help remind yourself about what you want to ask or do, or give it to your parent or healthcare worker to let them know you feeling a bit worried and tell them what you'd like to happen/don't want to happen.

If you want to print it off, you can fill it in and fold it up into a small piece of paper (like a leaflet!) to keep in your pocket or bag. This means other people won't see it unless you want them to.
